Alzheimer’s Society Hammersmith and Fulham provide a range of support services to both people with dementia and carers who are residents of the borough. The services include a day centre for people with dementia, peer support groups for carers and people with dementia, tri-borough Singing for the Brain group, and a one-to-one Dementia Adviser service for both carers and people with dementia.
Day centre:
A day support service for people with dementia who live in the community and would like to maintain their skills. The service aims to promote independence and provides different activities throughout the day such as poetry, reminiscence, art and crafts, outings, etc. transport and lunch are provided and there is a fee of £10 per day. A referral from a professional would be needed.
Peer Support Groups:
There are two bi-monthly support groups – one for people with dementia, and one for carers. The groups run on alternate Thursdays. Both groups are open and on-going; people are welcome to join all year round. The aim of the groups is to provide a safe and confidential environment where people can share with others in a similar situation and exchange experiences. Representatives from other organisations deliver information talks and the sessions are structured around the group members’ information and support needs. Referrals from professionals as well as self-referrals/family referrals are accepted. People need to call in advance to book their place.
Dementia Adviser:
A one-to-one information and advice service for people with dementia and their carers. The Dementia Adviser is able to see people at their homes as well and discuss their diagnosis, living well with dementia, services available and signposting/referrals to other relevant services and professionals. Referrals from professionals as well as self-referrals/family referrals are accepted.
